Pastoral Report Forms

ATTENTION: ALL THOSE UNDER MINISTERIAL APPOINTMENT

Please click on the link above to be routed to The Wesleyan Church denominational website to obtain your Annual Service Report. Be sure to choose the correct option for your appointment status (see below). You have a choice of opening the reports in an Excel Format (for completing the form on your computer and emailing to us) or in a Portable Document Format (PDF) (to be able to print out and fill in manually and mail it or scan it in and email to us). No faxing please! Please call or email the district office if you’re not sure which report to use. Thank you!

-Minister’s Annual – all ministers who are not serving as a lead or solo pastor, associate/assistant or as an evangelist

-Pastor’s Annual – lead or solo pastors only

-Assoc/Assist Pastor Annual – associate or assistant pastors only

-Ministerial Student Annual – only for students who have not been assigned a pastoral position by the district

-Retired Minister Annual – only for retired ministers who are not currently assigned a pastoral position by the district

-Lay Worker’s Annual & Statistical – two separate reports

-Evangelist’s Annual & Statistical – two separate reports

On the same page as the Annual Service Reports, in the upper right-hand corner, you can click on “Go to Statistical Reporting” to enter your local church statistics – you will have to log-in with your user name and password to access this site
